PUBLIC NOTICE:
REQUEST FOR PROPOSAL (RFP) for GERIATRIC DEPRESSION SCREENING
  

Snohomish County is soliciting responses from organizations interested in providing geriatric depression screening services to adults 60+ years of age in Snohomish County.

 

The Snohomish County Human Services Department, Office of Long Term Care and Aging has developed a Request for Proposal (RFP) information packet containing details on the scope of work to be performed and qualification standards for the awarded contract for this service.  The total funding available for Geriatric Depression Screening is $60,203. The amount is subject to appropriations. 

 

The period of performance is January 1, 2010 through December 31, 2010.  The information packet includes the Scope of Work to be Performed, Basic Terms and Conditions Addendum, and sample letter of intent.  The Letter of Intent must include required elements, as shown in the sample.

 

The RFP information packet (in PDF, 74 kb) is designed as the first step in the RFP process.  Any agency wishing consideration as a potential Snohomish County contractor must submit a Letter of Intent to Propose by Monday, August 17, 2009.  A facsimile is acceptable (425-388-7304) if it is received by the due date, and an original signed letter is received by mail with a postmark dated not later than August 17, 2009. 

 

An individual authorized to enter into a contractual agreement with Snohomish County must sign the Letter of Intent.  Failure to submit the Letter will disqualify agencies from any further consideration in this process.

 

If there is only one apparent proposer for the service, Snohomish County will declare the procurement to be “sole source” and enter into negotiations with the proposer to develop a contract for service.

If the County receives Letters of Intent indicating that more than one agency wishes to provide the service, a Request for Proposal (RFP) package will be sent to all competing agencies.  The RFP package will contain specific details of the entire process, specific services standards, and Technical, Cost and Management Proposal components.

Agencies will be notified by September 21, 2009 whether an RFP will be required or whether sole source provider contract negotiations can begin.
