Requesting Proof Of Insurance Letters
Snohomish County Departments needing proof of insurance letters for outside organizations should provide the following information to Risk Management:
1. Name, address, and contact person of the agency requesting proof of insurance.
2. Reason for the request. If it’s for a specific event provide:
a. The date(s) of the event
b. The location of the event
3) Date insurance verification is expected
