- Corner code number filled in (upper right corner of form)
- Form prescribed by Bureau of Surveyors and Maps Pursuant to the provisions of Section 5 (2), Chapter 50, Laws of 1973, (See sample, page)
- Two (2) original copies
- One copy is retained by County Auditor
- One copy is sent to the Bureau of Surveys and Maps in Olympia
- Legal Description
- Township, range, corner code number
- Type - DLC, HES, tract, mining claim, Indian allotment, Federal reserves, townsite
- History
- Description of evidence found
- Describe monument
